Abstract. The purpose of the study is to characterize the practices of preserving children’s lives and health in the first half and middle of the 19th century based on materials from Western Siberian cities. The article examines historical ideas about childbirth, breastfeeding, diseases, and early childhood mortality, the recommendations of F. V. Gebler (a physician of the Altai Mining District), the organization of smallpox vaccination in the cities of the Tomsk Governorate, individual instances of exile involvement in providing medical assistance and transferring medical knowledge, child care advice published in textbooks and journals, as well as living conditions of pupils in Siberian educational institutions. Selected materials from Eastern Siberia and the Perm Governorate are used for comparative analysis, while documents from the Altai Mining District serve to characterize administrative-medical policy, which also applied to factory and mining settlements. The scientific novelty lies in comparing medical prescriptions, administrative reporting, and source materials that allow evaluating the practical implementation of measures aimed at preserving children’s health. As a result, it was established that in the analyzed sources, medical recommendations and organized measures coexisted with customary caregiving practices and religious rituals. The implementation of organized measures depended on departmental support, the availability of physicians, institutional resources, and family attitudes toward the offered assistance. In the Altai Mining District, concern for children’s lives was linked to the reproduction of the departmental population, whereas materials from the Omsk military educational institution make it possible to trace the integration of a physician into the daily routine of pupils. By the middle of the 19th century, distinct practices and ideas had emerged, creating preconditions for the subsequent development of child hygiene, though they had not yet formed a stable system.
