Healthcare in the Tobolsk region amid social upheaval and changing political regimes (1917-1925)

Abstract.
The purpose of the study is to determine the stages, factors, and patterns of transformation of the healthcare system in the Tobolsk region in 1917-1925 within the context of the transition from the crisis collapse of pre-revolutionary infrastructure to a stable Soviet model of specialized medical care. The scientific novelty of the research lies in a comprehensive reconstruction of the mechanisms of transition from the pre-revolutionary to the Soviet healthcare model in the Tobolsk District, carried out using a wide range of archival documents from the Tobolsk Healthcare Department (Tobzdravotdel) introduced into academic discourse for the first time. The publication presents a retrospective assessment of the dynamics of the transition from the ruined pre-revolutionary healthcare model to the rigid mobilization structure of the Soviet healthcare department amid the typhus epidemic of 1919-1920. The activities of specialized sub-departments (sanitary, pharmaceutical, maternal and child health) are characterized. The influence of the West Siberian Uprising and the financial crisis of the NEP on the reduction of the medical network is analyzed. Key achievements of the mid-1920s are highlighted: the inception of the emergency medical service (1924), the implementation of A. G. Tutolmin’s advanced anti-tuberculosis doctrine, and child vaccination by V. V. Panchulidzeve. As a result, the phenomenon of the 1925 “personnel scissors” was revealed – a twofold increase in the number of doctors against the backdrop of a sharp erosion of rural medical assistants.
